Terms and Conditions

By signing or sending a prescription, work order (or substitution thereof) to Bennett Technologies, Inc., I agree to abide by all terms and polices listed below.

General Notice

  • As dental prosthetics are a custom piece, the cost of any prosthetics, devices, and/or appliances are unable to be refunded and all sales are final. If a dental prosthetic, device, and/or appliance does not meet our superior standards of function, quality, and aesthetics, Bennett Technologies, Inc. can issue a credit to the customer’s account upon return of all pieces if this occurs within the warranty period. 

  • All restorations are made in the United States and no foreign outsourcing is ever used. 

  • A copy of the dental laboratory work order must be retained in the dental laboratory office and the dentist’s office for a period of seven years. 

Remake & Warranty Policy

  • Bennett Technologies, Inc. has a five year warranty on most products. Crowns, bridges, and implant restorations are included in this warranty.

  • Veneers and custom shades have a one year warranty.

  • Please inquire with the lab for all other products. 

  • In the event that a restoration fails during the warranty period, Bennett Technologies will remake or adjust the product at no cost. 

Shipping Charges

  • Shipping charges vary by location of customer’s office and are charged the discretion of Bennett Technologies, Inc.

Payment

  • Statement balances are due on the 21st of each month. 

  • Balances over 60 days will be subject to a finance charge of 1.5% per month. This has an equivalent rate of 18% APR. All costs and attorney fees on balances that are referred to collection services will be covered by the receiving party of this invoice.

  • In the event that a balance exceeds 90 days, all cases and items sent to the lab will remain on property of Bennett Technologies, Inc. until the account is paid in full. 

  • The client agrees to be fully responsible for attorney fees and court costs for the enforcement of this provision.

  • The terms and policies contained herein represent all of the agreements, conditions and understandings between Bennett Technologies, Inc. and customer. In the event of a conflict between the terms of this document and any other document, the terms of and conditions of this document shall apply.

Agreement
